We are seeking dedicated Aged Care Clinical Assessors to join our multidisciplinary health team, working across Melbourne's Southern & Eastern suburbs.In this role, you'll deliver aged care assessment services to older Australians, enabling them to remain living independently at home.About You:Preferred candidates will have:Experience conducting home support or comprehensive aged care assessments within the single assessment systemAbility to work independently and as part of a teamStrong problem-solving skills and ability to follow relevant guidelinesCurrent Australian driver's license with a reliable, fully insured vehicleValid National Criminal History Check and Working with Children Check (or willingness to obtain)A willingness to travel within a 50km radiusWhat You'll Do:Conduct face-to-face assessments to determine eligibility for My Aged Care supportsReview previously assessed clientsDevelop client-centered support plans using a wellness and reablement approachLiaise with referral sources and community organisations to secure supports for clientsEnsure timely, accurate documentation via the My Aged Care online portalAbout Us:Since ****,
