About the role
As the Diabetes Educator & Cardiac Rehabilitation CNS, you will provide specialist clinical leadership and individualised, case-managed care across Diabetes Management and Cardiac Rehabilitation within Moree Health Services.
Your role will include both community and acute support, with opportunities to provide diabetes management support to clients across the broader Mehi region.
You will be supported by the community Health service manager, as well as the broader multi-disciplinary allied health team and aboriginal health unit.
Additionally, you will have access to Online learning resources to support your professional development and mentoring to assist you to develop your cardiac rehabilitation skills.
About you
We are looking for a motivated and autonomous clinician who is passionate about diabetes management and committed to improving health outcomes for people living in rural and regional communities.
You will bring:
Current registration as a Registered Nurse with AHPRA.
A postgraduate qualification in Diabetes Management and-or Chronic Care, with at least three years' postgraduate experience.
Demonstrated clinical knowledge and experience in diabetes management.
Experience in cardiac rehabilitation is highly regarded; however, this role provides an excellent opportunity for an experienced diabetes clinician to develop specialist skills in cardiac rehabilitation.
A current unrestricted NSW driver's licence and willingness to travel for work purposes, including outreach to surrounding communities, is required.

Based on a minimum 18-month commitment to the position or the agreed duration of your contract.
All payments to part-time and contract employees will be made on a pro-rata basis in accordance with the terms of your contract, including any approved extensions.
If your contract ends before the agreed completion date, you may be required to repay a portion of the incentive.
