Daily Responsibilities:
n
n
Conduct regular health assessments and monitor disease progression and treatment responses.
n
Develop care plans and coordinate with healthcare professionals.
n
Anticipate and manage changing health needs of patients.
n
Support patients and families by responding to inquiries and feedback.
n
Maintain accurate clinical documentation, including referrals, in a timely manner.
n
Administer prescribed medications and treatments.
n




Participate in or lead chronic disease management programs.
n
n
The Successful Candidate:
n
n
Experience in the chronic disease field or postgraduate qualification in chronic disease.
n
Bachelor of Nursing and AHPRA registration.
n
Full working rights within Australia.
n
Willingness to work in a regional setting and embrace adventure.
n
Ability to pass a national police clearance.
n
Highly organized, capable of working under pressure, with strong time management skills.
n
