Bundaleer Home Care, part of Apollo Care, is seeking a dedicated Clinical Care Partner to join our growing team. This is an exciting newly created opportunity to provide person-centred clinical care that helps older Australians remain independent and continue living safely in their own homes.

About The Role
n
As a Clinical Care Partner, you will work closely with clients, families and the wider care team to deliver high-quality clinical support across a range of home care programs. No two days are the same, with responsibilities including clinical assessments, care planning, nursing interventions and supporting clients to achieve their goals.
n

Key Responsibilities
n

Conduct comprehensive clinical assessments and reviews
n
Develop and maintain person-centred care plans
n
Deliver nursing interventions within your scope of practice
n
Support clients with wound care, medication management, chronic disease management, diabetes care and other clinical needs
n




Monitor client wellbeing and identify clinical risks
n
Coordinate referrals to GPs, allied health and specialist services
n
Provide clinical guidance and support to Care Partners and Support Workers
n
Maintain accurate documentation and compliance with funding requirements
n

About You
n
n
Current AHPRA registration as a Registered Nurse
n
Current First Aid and CPR certification
n
Current driver's licence and access to a reliable insured vehicle
n
NDIS Worker Screening Check (or willingness to obtain)
n
At least 1 year of experience in community nursing, home care or a similar clinical setting
n
Strong assessment, clinical decision-making and communication skills
n
A genuine passion for person-centred care
n
