We are seeking a passionate, experienced Senior Social Worker to join our team at the SMHS Older Adult Health Hub.
n
This is a full‐time, fixed‐term position until 28 June ****, with possibility of extension(s) and/or permanency.
n
The Older Adult Health Hubs are a new model of care, providing safe and quality care for older adults through integrated, community‐based intervention. This specialist‐led older adult hub delivers and integrates with services that focus on proactive and responsive care, risk prediction and early intervention.

The SMHS Older Adult Health Hub:
n
n
Optimises older adult independence through maintaining and improving function, growing health literacy, supporting choice autonomy, and supporting carers
n
Provides coordinated, comprehensive multidisciplinary care
n
Supports liaison between primary care and SMHS services
n
n
About the role: As part of a multidisciplinary team you will plan, implement,



coordinate and evaluate Social Work services to patients in the OAH Hub using advanced Social Work clinical practice skills. You will also plan, implement and evaluate patient safety and quality of care. As a Senior Social Worker you will ensure practice is in accordance with the Australian Association of Social Workers Practice Standards and Code of Ethics and SMHS policies and guidelines.

Qualifications:

Valid working visa (for fixed‐term appointments) or Australian citizen, permanent resident, or visa that allows indefinite work in Australia (for permanent appointments).

Relevant social work experience and professional registration.
n
