Disability Support Worker (South Eastern Suburbs, Melbourne)

About the Role
n
As a Disability Support Worker, you will deliver person-centred, rights-based, trauma-informed and evidence-informed supports that empower participants to live meaningful and independent lives. Working across Supported Independent Living (SIL) and Community Services, you will promote supported decision-making, Active Support, inclusion and participant choice while ensuring safe, high-quality supports are delivered every day.

Working as part of a collaborative team, you will deliver supports that are person-centred, rights-based and aligned with contemporary disability practice, ensuring participants are empowered to exercise choice, control and supported decision-making.
n
This role offers opportunities to work across both Accommodation and Community Services. Your responsibilities will include:
n
Providing high-quality personal care and provide person-centred supports that maximise independence through Active Support principles.
n
Supporting participants to achieve their individual goals and maximise independence
n
Encouraging participant choice, autonomy and supported decision-making.
n
Assisting participants to access community, recreational and social opportunities.
n
Implementing Behaviour Support Plans, Risk Assessments and participant-specific support strategies.
n
Supporting participants with complex support needs, including behaviours of concern and high-intensity supports where trained.
n
Administering or assisting with medication in accordance with organisational policies and training.
n
Recognising and responding to changes in participant health, wellbeing or behaviour and escalating concerns appropriately.
n
Maintaining accurate and timely documentation, including progress notes and incident reports.
n
Working collaboratively with families, guardians, clinicians and multidisciplinary teams to achieve positive participant outcomes.

About You
n
You are passionate about making a genuine difference and committed to delivering contemporary disability supports that place participants at the centre of everything you do.
n
You will demonstrate:
n
A commitment to person-centred, rights-based and trauma-informed practice.
n
An understanding of Active Support, supported decision-making and participant choice.
n
Experience supporting people with complex support needs and/or behaviours of concern.
n
Excellent communication, interpersonal and relationship-building skills.
n
Strong observation, problem-solving and decision-making abilities.
n
Professionalism, integrity and sound judgement.
n
A commitment to safeguarding, participant rights and professional boundaries.
n
The ability to work independently while contributing positively within a multidisciplinary team.
n
A willingness to participate in ongoing learning, reflective practice and career development.
n
Minimum 2 years' experience supporting people with disability, including participants with complex support needs and/or behaviours of concern.
n
Qualification in Disability, Community Services or a related discipline.
n
Current NDIS Worker Screening Clearance.
n
Current Working with Children Check (Employee).
n
Current National Police Check (or willingness to obtain).
n
Current Provide First Aid (HLTAID011) and Provide CPR (HLTAID009).
n
Current Victorian Driver Licence.
n
Reliable motor vehicle with comprehensive insurance.
n
Availability to work a flexible rotating roster, including weekends, sleepovers and public holidays where required.
