About the role
n
You will provide individual psychosocial support in participants' homes and communities across the Mornington Peninsula and surrounding areas.
n
Each participant and shift is different. A typical day may involve:
n
n
- Supporting someone to establish or maintain their daily routine
n
- Attending an appointment or community activity
n
- Breaking an overwhelming task into achievable steps
n
- Building confidence with everyday responsibilities
n
- Supporting social connection and community participation
n
- Reinforcing an agreed strategy recommended by an occupational therapist or another member of the participant's care team
n
- Providing calm, practical support during periods of distress, overwhelm or reduced functioning
n
- Recognising and documenting changes in functioning, support needs or risk
n
n
You will work alongside participants, helping them build skills and confidence without unnecessarily taking over.
n
This is a non-clinical role. You will complement, rather than replace, the work of allied health and clinical professionals.
n

Your observations, communication and case notes also help the participant, LIU and their wider care team understand what support is required, what is changing and which strategies are helping.

What you will contribute

In this role, you will:

- Receive relevant guidance from allied health practitioners and translate it into practical support

- Observe and document functional changes, progress, barriers and support needs
n
- Complete accurate, respectful and timely case notes
n
- Communicate professionally with participants, colleagues and relevant stakeholders
n
- Raise concerns early through the appropriate internal process
n
- Contribute to continuity across the participant's support team
n
- Participate in reflective practice, guidance and ongoing learning

Experience and requirements
n
We welcome applications from people with relevant experience in:
n

- Psychosocial disability support
n
- Mental health or recovery-oriented services
n
- Disability or community support
n
- Community services
n
- Allied health environments
n
- Other person-centred roles involving complex support needs
n
n
Study or qualifications in occupational therapy, psychology, mental health, social work, allied health or community services will be highly regarded, but these are not the only ways to demonstrate suitability.
n
Applicants must hold, or be willing and eligible to obtain:
n

- NDIS Worker Screening Check
n
- Working with Children Check, where required
n
- Current First Aid and CPR
n
- Current Australian driver licence
n
- Access to a reliable and appropriately insured vehicle
n
- Australian working rights
n
- Any additional training or checks required for assigned services
n
n
Clear written communication and the ability to complete timely case notes are essential.
