Community Nurse: Aged Care: Mildura & Loddon Mallee Region

McArthur are working in partnership with Uniting AgeWell, a not-for-profit organisation who pride themselves in providing excellent services to our Ageing Community. As a Community Aged Care Nurse, you will deliver high-quality clinical care and case support to older people receiving services under the Support at Home program.

Working across Mildura and surrounding communities, you will build meaningful relationships with clients and their families while delivering person-centred care that promotes health, well being, and independence.

Key Responsibilities

n

n
- Conduct clinical assessments and develop care plans.
n
- Monitor client health and well being in the home setting.
n
- Provide wound care, medication management, and chronic disease support.
n
- Liaise with GPs, allied health professionals, and support workers.
n
- Identify changes in client needs and coordinate appropriate services
n




- Maintain accurate clinical documentation and compliance standards.
n

About You

n

n
- Current AHPRA registration Div 1 RN.
n
- Experience in community nursing in rural communities.
n
- Strong clinical assessment and communication skills.
n
- Ability to work independently and manage a varied caseload.
n
- Current Driver's Licence and reliable vehicle.
n
- National Police Check and Working with Children Check (or willingness to obtain).
n
- Must be available to work Monday - Friday business hours.
n
