About the Role
n
As a Care Partner you will manage a portfolio of aged care clients receiving home care services, ensuring they receive high-quality, person-centred support tailored to their goals and needs.
n
You will work closely with clients, families, support workers, allied health professionals, and internal staff to coordinate outstanding care outcomes.
n
This is more than a case management role — it is an opportunity to become a trusted partner to clients and families, helping improve lives every day.
n
Location: Sydney, NSW
n Hours: Monday to Friday - Flexible start and finish times available
n
Key Responsibilities
n
- Conduct client assessments and care planning
n
- Develop, review, and update individual care plans
n
- Coordinate supports and services to meet client goals
n
- Manage client budgets and funding utilisation
n
- Monitor client wellbeing and respond to changing needs
n
- Build trusted relationships with clients and families
n
- Maintain accurate case notes and compliance documentation
n
- Manage incidents, feedback, and complaints
n
- Liaise with health professionals and external stakeholders
n
- Ensure services align with aged care quality standards
n
About You
n
You are a compassionate and confident nurse who enjoys building relationships, solving problems, and making a genuine difference.
n
Essential Requirements
n
- Enrolled Nurse qualification
n
- Current AHPRA registration
n
- Experience in aged care, home care, community nursing or case management
n
- Strong communication and relationship-building skills
n
- Organised with excellent time management
n
- Confident using Microsoft Office and care management systems
n
- Current driver's licence and reliable vehicle
n
- First Aid & CPR Certificate
n
- Current Police Check and Working with Children Check
n
- NDIS Worker Orientation Module and NDIS Screening Check
n
- Full Australian working rights
n
Highly Regarded
n
- Home Care Package experience
n
- Knowledge of aged care reforms / Support at Home model
n
- Dementia or complex care experience
n
