Key Responsibilities

n

n
- Build genuine, trusting relationships with clients, families, and representatives, becoming their go-to support
n
- Conduct regular care reviews (in person or via phone)



to ensure services continue to meet client needs and expectations
n
- Collaborate with caregivers, families, and health professionals to ensure care plans are personalised and effective
n
- Maintain strong community connections by engaging with local services, health providers, and referral networks
n

About you

n

n
- A tertiary qualification in a health-related field (e.g. social work, allied health) with at least 1 year of experience working with seniors and Home Care Packages/Support at Home Packages.
n
- Excellent organisational skills with the ability to juggle priorities and adapt to changing needs
n
- You thrive in a team environment and are willing to offer assistance when needed.
n
- Strong communication skills, both written and verbal, with proficiency in Microsoft Office
n
- A valid Australian Driver's Licence and the flexibility to travel when required
n
