Job Description
Position: Home Care Support Worker
n Location: Tanah Merah and surrounding areas
n Job Type: Casual
n Salary: Starting from $36.08 per hour

About the Role
n
We are seeking compassionate and dedicated Home Care Support Workers to join our team and provide exceptional care and assistance to individuals living with mental health needs.
n
In this role, you will support participants with activities of daily living (ADLs) and community access, helping them lead fulfilling, independent and enriched lives.
n
You will work closely with participants to assist them with all aspects of their daily routines, from personal care to engaging in social and community activities, with a focus on maximising independence and promoting choice and control.
n
Key Responsibilities
n
n
- Assist participants with activities of daily living, including personal care, mobility support such as hoisting, and medication management.
n
- Support participants with community access, including attending appointments, social events and recreational outings.
n
- Foster independence by encouraging participants to develop skills in everyday tasks.
n
- Provide emotional and social support while promoting participant empowerment and wellbeing.
n
- Maintain accurate documentation, including shift notes, incident reports and other required records.
n
- Collaborate with other team members to ensure participants receive holistic and high-quality care.
n
- Adhere to all NDIS standards, policies and procedures to ensure quality and safety in service delivery.
n
n
Approximate Shift Times
n
Shifts are available Monday to Sunday across a rotating 24/7 roster:
n
n
- Morning shifts: 7:00 am–3:00 pm
n
- Afternoon shifts: 3:00 pm–11:00 pm
n
- Night shifts: 11:00 pm–7:00 am
n
- Sleepover shifts: 3:00 pm–7:00 am
n
n
Supplementary shifts may also be available outside the above shift times for employees seeking additional/other hours.
n
Key Requirements
n
n
- Previous experience in disability support, particularly supporting individuals with mental health needs and challenging behaviours, is highly desirable.
n
- Certificate III or IV in Disability, Individual Support or a similar qualification, or willingness to obtain.
n
- Ability to work effectively both independently and as part of a team.
n
- Robust communication and interpersonal skills, with a compassionate and person-centred approach.
n
- Valid driver's licence and access to a reliable vehicle, as participant transportation may be required.
n
- Current First Aid and CPR certificates, or willingness to obtain.
n
- Paid NDIS Worker Screening Card, or willingness to obtain.
n
