ABOUT THE ROLE

Working within a multidisciplinary team with a practice nurse, integration coordinator and alongside Soul Hub allied health professionals, you will deliver accessible, trauma-informed and culturally safe primary health care to Soul Hub Guests. This is a unique opportunity to help shape a current model of care from the ground up, one that brings medical, mental health and social support together to reduce barriers to access and improve health equity for a highly vulnerable population. DUTIES

n

n
- Provide holistic, primary care addressing physical health, mental health, chronic disease and psychosocial needs
n
- Conduct comprehensive assessments, diagnose and manage acute and chronic conditions, including complex comorbidities
n
- Partner closely with the Practice Nurse, Integration Coordinator and wider team on collaborative care planning and case management
n
- Deliver trauma-informed, non-judgmental care that responds to the impact of homelessness, stigma and lived experience
n
- Contribute to clinical governance, quality improvement and evaluation of the Soul Clinic
n

SKILLS & EXPERIENCE

n

n
- Current unrestricted AHPRA registration as a General Practitioner
n
- Vocational registration (FRACGP or equivalent)
n
- Demonstrated capability or experience in trauma-informed primary care
n




- Skills and experience managing mental health conditions and/or substance use presentations
n
- Proven ability to work effectively within a multidisciplinary, integrated care team
n
- Excellent interpersonal, communication and consultation skills
n
- Openness to using and adapting to AI-enabled clinical tools within an ethical framework
n

Desirable

n

n
- Experience working with complex and/ or vulnerable populations
n
- Experience in integrated care or community-based health settings
n

n

Special Conditions

- Current NSW Working with Children Check
n
- National Police Check suitable for work with vulnerable persons
n
- Current Professional Indemnity Insurance
n
- CPR completed within the triennium (3 yrs)
n
