Job Description

Southern Cross Care (NSW & ACT) is seeking a Home Care Service Advisor in Merrylands. This role involves conducting client assessments, care planning, coordinating service delivery, and managing funds and resources within Home Care Programs.

You will maintain comprehensive documentation, ensure compliance with aged care standards, and collaborate with Home Care Employees, the Quality team and Allied Health professionals to deliver high-quality, client-centred care across Sydney locations.
