Job Description
Join the Residential Transition Care Program as a Senior Physiotherapist, delivering advanced clinical care, leading multidisciplinary rehabilitation and supporting older patients to achieve safe and successful transitions from hospital to home or aged care.

About the role
n
Temporary full time opportunity toprovide high level physiotherapy clinical services within the multidisciplinary Residential Transition Care program (RTCP) of Community and Oral Health, Metro North Health. This service provides short-term support and case management to assist older people transition from hospital to home or hospital to a residential aged care facility. RTCP uses a co-ordinated multidisciplinary approach to support patients with a low intensity therapy program to further improve or maintain their physical, cognitive and psycho-social functioning. The Allied Health team in Residential Transition Care operates from 0800 to 1630 hours Monday to Friday excluding public holidays

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

- Deliver high level physiotherapy clinical services to patients of RTCP, in accordance with prescribed professional and ethical standards.
n
- Actively participate in the multidisciplinary team, to independently provide clinical advice regarding physiotherapy service delivery,
working cooperatively and effectively within the team and with patients to provide high quality care.
n
- Provide education, mentoring and clinical practice supervision for less experienced practitioners, clinical assistants, students, support staff, and other health professionals.
