We are seeking compassionate and dedicated Community Support Workers to support NDIS participants across Melbourne's Western and Northern suburbs. Through person-centred support, you'll help participants achieve their goals, build confidence, develop independence, and engage meaningfully in their communities.

About the Role

n

As a Community Support Worker, you will provide person-centred support that enables participants to live independently, achieve their goals, and engage with their community.

n

n
- Supporting participants with daily living activities
n
- Assisting with community access and social outings
n

n

Transporting participants to appointments, education and activities

n

n
- Encouraging independence and life skills development
n
- Providing personal care support where required
n
- Promoting participant choice, dignity, and inclusion
n
- Maintaining accurate documentation and progress notes
n

n

About You:

- Caring, reliable, and passionate about helping others

- A strong communicator with a positive attitude
n
- Able to work independently and as part of a team
n
- Adaptable and committed to delivering high-quality support
n
- NDIS Worker Screening Check (or willingness to obtain)
n
- NDIS Worker Orientation Module Certificate
n

n

Current Working with Children Check -Employee

n

n
- Current First Aid & CPR Certificate
n
- Current Australian Driver Licence
n
- Reliable vehicle with comprehensive insurance
n
- Current National Police Check
n
- Smartphone and basic computer skills
n
- Certificate III or IV in Individual Support, Disability, Community Services, Mental Health, or similar
n
- Previous experience in disability support, aged care, mental health, or community services
n
