About the role

n

City Heart Medical is currently looking for a full-time Registered Nurse to work with our dedicated team of nurses, staff, and doctors providing the highest quality care in the role of Chronic Disease Management Nurse.

n

In this role,



you will be expected to work both autonomously and collaboratively to deliver great patient care. Duties will include all aspects of General Practice nursing with a strong focus on Care Plans and Reviews, Health Assessments, Patient Education and Follow Up etc.

What we're Looking For:

n

n
- You're a nurse with AHPRA registration and full working rights in Australia (no sponsorship available)
n
- You're experienced in GP Chronic Condition Management Plans and Health Assessments
n
- You're passionate about patient care
n
- You hold a current CPR certificate - although assistance can be offered here if it has recently expired.
n

n

Remuneration will be based on experience.
