We are recruiting a Registered Nurse who will work in a small, collaborative clinical team to deliver high-quality clinical care and care coordination across our community programs. This role will be based in the Illawarra region, with occasional travel to the Shoalhaven region required.

- Payrate: $50.68/hour + NFP Salary Packaging + KM reimbursement
n
- Hours per week: Full time, 38 hours/ week. We are open to a part time role for the right candidate.
n

- Location: Illawarra, with occasional travel to the Shoalhaven region

What You'll Be Doing
n
n
- Deliver direct, comprehensive nursing care and individual case management to aged care and NDIS participants in their homes and community
n
- Conduct nursing assessments and develop, implement and review car plans and Advanced Health care plans in collaboration with clients and their families/ carers.
n
- Support direct care workers through education and guidance to maintain best practice clinical standards
n
- Coordinate services across health providers and community stakeholders to meet participants changing needs.
n
n
What we're looking for
n
n
- Registered Nurse with current AHPRA registration
n
- Demonstrated experience working with complex clients in aged care, disability, or community settings
n
- Demonstrated clinical leadership and mentoring capability
n
- Current NSW Driver's Licence
n
- Current First Aid Certificate
n
n
Desirable (highly regarded)
n
n
- Advanced clinical assessment and case management experience
n
- Knowledge of Home Care Packages (HCP) and Support at Home program
n
- Experience in palliative care, dementia, or end‐of‐life care
n
- Understanding of Aged Care Quality Standards and clinical governance
n
n
