Job Description

A Bouaraphan Pty Ltd Toowoomba Care Support Services is seeking a dedicated and experienced Disabilities Services Officer to join our growing team in Toowoomba. The successful applicant will work with clients, families and support networks to assess individual needs, develop and coordinate support arrangements, and monitor outcomes across home and community settings. The role will involve case management, service coordination, capacity building and liaison with disabilities, health, welfare and community service providers.

Competitive salary of $79,500–$85,000 + superannuation.

Task & responsibilities

- Assessing clients' needs, goals, risks, strengths and barriers and identifying appropriate support strategies.
- Interviewing clients and consulting with families, carers and other support networks to understand individual circumstances and desired outcomes.
- Planning, developing, implementing and reviewing individual support and capacity-building programs in consultation with clients and relevant stakeholders.
- Coordinating disability, health, welfare and community services to support clients' individual goals and ongoing participation.
- Monitoring and reviewing client progress, identifying changes in needs or circumstances, and recommending adjustments to support arrangements where required.
- Referring clients to allied health professionals, government agencies, community organisations and other relevant services.




- Liaising with families; NDIS planners, support coordinators and other relevant representatives; community organisations; allied health professionals and service providers to facilitate coordinated support.
- Maintaining accurate case-management records, assessments, support plans, progress reports and other required disability-services documentation.
- Identifying and responding appropriately to risks, incidents, changes in client circumstances and complex situations, and escalating matters where required.
- Supporting clients to build independence, develop capacity and participate in community and social activities.
- Providing incidental assistance with daily living activities or medication assistance where required and within the scope of the role, organisational policies and relevant requirements.
- Participating in training, supervision, performance reviews and ongoing professional development.
- Following TCSS policies, procedures and relevant disability service standards and requirements.

Qualification & experience

- An AQF Diploma or higher qualification in Disabilities Services, Community Services, Social Welfare or another closely related field, together with at least one year of relevant experience in disability services, case management, support coordination or service planning.




- In the absence of a relevant qualification, at least three years of directly relevant experience in a comparable position is required.
- Good knowledge of the NDIS and the disabilities services sector.
- Experience maintaining case records, assessments, progress reports and other disability-services documentation.
- Current Blue Card and Yellow Card / NDIS Worker Screening Check / QLD Working with Children Check (where child-related work is involved), or willingness to obtain the required checks.
- Australian driver's licence.
- Right to work in Australia.
- Solid verbal and written communication skills, with the ability to communicate effectively with clients, families, support networks and service providers.
