About the Role
n
At Health and Community Services (HCS), we are seeking a skilled, compassionate and motivated Behaviour Support Practitioner to join our growing multidisciplinary team.
n
In this role, you will work with NDIS participants who require behaviour support, using evidence-based and person-centred approaches to improve quality of life, increase independence and reduce behaviours of concern.
n
You will work collaboratively with participants, families, support teams and other allied health professionals to develop and implement high-quality Positive Behaviour Support Plans in accordance with NDIS requirements.
n
Key Responsibilities
n
- Conduct comprehensive Functional Behaviour Assessments (FBAs) to understand behaviours of concern, contributing factors and support needs.
n
- Develop Interim and Comprehensive Behaviour Support Plans that are evidence-based, person-centred and consistent with NDIS requirements.
n
- Develop practical behaviour support strategies aimed at improving participants' quality of life, independence and community participation.
n
- Provide behaviour support interventions to participants across home, community and supported living environments.
n
- Provide training, coaching and ongoing guidance to families, carers, support workers and other stakeholders in implementing behaviour support strategies.
n
- Monitor participant progress and evaluate the effectiveness of behaviour support strategies.
n
- Review and update Behaviour Support Plans based on participant outcomes, data and changing needs.
n
- Work collaboratively with Occupational Therapists, Support Coordinators, psychologists, nurses, support workers and other allied health professionals.
n
- Support the reduction and elimination of restrictive practices wherever possible and ensure appropriate reporting and documentation where restrictive practices are used.
n
- Maintain accurate, timely and compliant clinical documentation, progress notes and reports.
n
- Participate in case conferences, stakeholder meetings and multidisciplinary team discussions.
n
- Maintain a strong understanding of the NDIS Quality and Safeguards Commission requirements relating to behaviour support and restrictive practices.
n
- Contribute to continuous improvement, clinical governance and best-practice service delivery within HCS.
n
About You
n
You are a passionate and clinically confident practitioner who enjoys working collaboratively with participants and their support networks.
n
You have strong assessment, communication and report-writing skills and are comfortable managing a diverse community-based caseload.
n
Essential
n
- Relevant tertiary qualification in Psychology, Social Work, Occupational Therapy, Nursing, Behavioural Science, Counselling, Disability, Education or another relevant discipline.
n
- Suitable experience and/or qualifications to practise as an NDIS Behaviour Support Practitioner.
n
- Registration or suitability with the NDIS Quality and Safeguards Commission as a Behaviour Support Practitioner, where applicable.
n
- Understanding of Positive Behaviour Support and functional behaviour assessment.
n
- Strong clinical reasoning, assessment and report-writing skills.
n
- Understanding of behaviours of concern and restrictive practices.
n
- Excellent communication and stakeholder engagement skills.
n
- Ability to work independently and effectively manage a diverse caseload.
n
- Experience working within the NDIS framework is highly desirable.
n
Requirements

- Current Driver's Licence.
n
- Reliable vehicle and appropriate vehicle insurance.
n
- Working with Children Check.
n
- NDIS Worker Screening Check, or willingness to obtain one — support will be provided.
n
- Current police check where required.
n
- Ability to undertake community-based appointments across participants' homes and other service settings.
n
