We're growing our Clinical Care Team.
n
As more people choose Right at Home Southwest Victoria, we're expanding our Clinical Care Team and are looking for an experienced Registered Nurse to join us as a Clinical Care Team Leader.
n
This newly created role offers the opportunity to provide clinical leadership, mentoring and support while working alongside our Care Manager to deliver secure, high quality and person-centred care across our community.

About the Role
n
As the Clinical Care Team Leader, you will:
n
n
- Provide clinical leadership, mentoring and support to the Clinical Care Team.
n
- Assist with complex clinical assessments, reviews and care planning.
n
- Provide clinical guidance and support in complex client situations.
n
- Support quality improvement, clinical governance and risk management activities.
n
- Maintain a reduced client caseload while supporting clinical consistency across the organisation.
n
- Work collaboratively with the Care Manager to achieve positive client outcomes.
n
n
About You
n
You'll be an experienced Registered Nurse who enjoys working collaboratively, sharing your knowledge and supporting others to succeed.
n
You'll also have:
n
n
- Current AHPRA registration as a Registered Nurse.
n
- Experience in community aged care, home care or a related clinical environment.
n
- Excellent clinical assessment and decision-making skills.
n
- Strong communication and relationship-building abilities.
n
- A passion for mentoring and supporting colleagues.
n
- A commitment to person-centred care and continuous improvement.
n
n
