Job Description

The Program Coordinator role at Queen Elizabeth Centre is based at the Preston office, four days a week (Mon–Thu), for 12 months. You will lead a small multidisciplinary team delivering home-based care to families with young children and provide face-to-face home support.

You will mentor staff, manage a small caseload, review referrals, and coordinate with clinicians to ensure secure, evidence-informed, person-centred care within QEC's Model of Care.
