Job Description

Icon Group, via Slade Pharmacy Warragul, is seeking a Clinical Pharmacist to deliver hospital-based clinical services on site at West Gippsland Hospital. The role covers multiple wards and focuses on medication reconciliation, clinical reviews and discharge support.

You will work with doctors, nurses, and allied health staff to optimise therapy, promote secure medication use, and provide expert advice. The ideal candidate is AHPRA-registered with Australian hospital or community pharmacy
