We're seeking a Registered or Enrolled Nurse to join our Wyoming Medical Centre, where you'll support GPs and patients through chronic condition management and preventative healthcare. You'll be part of a great, supportive team that values collaboration, culture, and patient outcomes. Part-time and full-time opportunities available.

What You'll Do:

- Prepare Care Plans and Team Care Arrangements
n
- Support patients in managing chronic conditions and improving health behaviours
n
- Conduct health assessments and promote preventative care
n
- Collaborate with GPs and multidisciplinary teams
n
- Manage administrative tasks related to CCM appointments
n

n

About You:

n

n
- AHPRA registered Enrolled or Registered Nurse
n
- Prior experience in Chronic Condition Management preferred
n
- Strong communication and organisational skills
n
- Friendly, proactive,



and patient-focused approach
n
