Job Description
This is a fixed-term, part time position (19 hours per week), based in Moruya until 30 June 2027.

The Care Navigator, Medicare Mental Health Centre Moruya works collaboratively with consumers to connect with services and supports in the community, facilitating person-centred and integrated care.
n
The role includes concierge duties, which will allow you to work collaboratively with consumers to connect with services and supports in the community, facilitating person-centred and integrated care.

Skills & Experience
n
n
- Cert IV in community services or relevant discipline, and/or extensive experience in mental health, health community services and willingness to work towards Cert IV.
n
- Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or alcohol or other drugs concerns.
n
- Demonstrated understanding of supports and services available in the local and regional communities.
n
- Demonstrated ability to engage with consumers and their families or carers to provide care navigation and referral support.
n
