Australian Unity is seeking a Care Partner to support Elders in Aboriginal Home Health in NSW, developing holistic care plans and coordinating services with families, clinical teams and community providers to enable secure, person-centred care that honors culture and land.

This full-time, hybrid role is based between Coffs Harbour - Grafton and surrounding areas, with office/home work and client visits to age well at home and on Country.
