Job Description

Proveda is seeking a dynamic Care Coordinator to help older people, people with disability and mental health needs navigate the NDIS, Home Care and related services in the Northern Sydney region.

n

Reporting to Practice Managers, you will deliver impartial information and guidance, support planning and service navigation, and connect clients with appropriate supports. The role is permanent part-time (4 days weekly) with flexible working after training and a cooperative team.
