Schedule: Saturday Day/Evening Doubles AND Full-Time NOC August / September

Location: Avamere Rehab of Clackamas, 220 E Hereford St, Gladstone, OR 97027

Job Summary

n

Licensed Practical Nurse provides basic nursing care, including vital signs, medication administration, wound care, and assisting residents with daily activities, under the supervision of a Registered Nurse (RN)

n

Essential Duties and Job Responsibilities

n

Basic Nursing Care:

n

n
- Vital Signs:Accurately measure and record vital signs like temperature, pulse, blood pressure, and respirations.
n
- Medication Administration:Administer medications as prescribed by a physician, following facility protocols and under the supervision of an RN.
n
- Wound Care:Perform wound care, including changing dressings and monitoring for signs of infection.
n
- Hygiene and Mobility:Assist residents with personal hygiene, dressing, bathing, and mobility.
n
- Nutrition and Hydration:Ensure residents receive proper nutrition and hydration, assisting with feeding if necessary.
n

Patient Monitoring:

n

- Patient History:Record a patient's history and current condition.
n
- Observation:Observe and document changes in a resident's condition, reporting any concerns to the RN.
n
- Communication:Communicate effectively with residents, families, and other healthcare professionals.
n

n

Collaboration and Supervision:

n

n
- RN Supervision:Work under the supervision of an RN, following their guidance and instructions.
n
- CNA Supervision:Supervise certified nursing assistants (CNAs) in providing basic nursing care.
n
- Teamwork:Collaborate with other healthcare professionals to ensure residents receive comprehensive care.
n
- Other Duties:

- Patient Safety:Maintain a safe environment for residents, following infection control protocols and other safety guidelines.
n
- Documentation:Accurately document all care provided, including medications administered, vital signs, and any observations.
n
- Emotional Support:Provide emotional support and comfort to residents and their families.
n

Requirements and Qualifications

n

n
- Must possess a nursing degree from an accredited college or university.
n
- Must possess a current, unencumbered, active license to practice as an LPN in this state.
n
- Must maintain and have an active CPR/BLS during employment.
n
- Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.
n
- Experience in electronic medical records documentation.
n
