Echuca Regional Health is seeking an experienced Social Worker to join our Community Rehabilitation Program. You will provide person-centred assessment, care planning, counselling, advocacy, and service coordination to clients and their families within the community.
As part of a multidisciplinary team, you will support clients to achieve rehabilitation goals and maintain independence, contributing to their transition back to community-based services.
