About your recent role

n

Delivermarket-leading quality RMMRs and HMRs

n

Conduct QUM services including attending MAC meetings, conducting nurse/carer education and advising on medication use and storage.

n

Maintain and grow relationships with facility staff and GPs

n

Must haves

n

Credentialing certificateto perform medication management review services

n

Arecognised undergraduate pharmacy degree or equivalent, and unconditional AHPRA registration as a pharmacist in Australia

n

Professional indemnity insurance policy

n

WorkCover insurance policy required for Corporate Contractors (if applicable)

n

Australian Business Number (ABN)

n

Australian citizen or hold permanent residency status in Australia

n

Appropriate police clearance and statutory declaration

n

Annual flu vaccination (unless medical exemption)

n

COVID 19 vaccination (unless medical exemption)

n

Working with Children Check and/or NDIS Clearance (if required)

n

Proficiency in appropriate computing applications

n

Excellent clinical skills

n

Strong organisational, planning and time management skills

n

An interest in relationship building with healthcare professionals and others working in the aged care industry

n

Effective communication skills; listening, written and verbal; willing and able to communicate with aged care residents and their relatives as well as colleagues and other healthcare professionals and aged care staff
