Job Description
Assist participants to understand and implement their NDIS plans.
n
Coordinate and connect participants with appropriate service providers.
n
Support participants to achieve their goals and build independence.
n
Monitor supports, budgets, and service delivery to ensure quality outcomes.
n
Liaise with participants, families, carers, the National Disability Insurance Agency (NDIA), and other stakeholders.
n
Prepare participants for NDIS plan reviews and obtain supporting reports and assessments.
n
Advocate for participants and coordinate multidisciplinary supports.
n
Maintain accurate case notes, reports, and participant records.
n
Identify and manage risks, safeguarding concerns, and changes in participant needs.
n
Ensure services are delivered in accordance with the NDIS Quality and Safeguards Commission Practice Standards, organisational policies, and a person-centred approach.
