The need-to-know
We're looking for an experienced Team Leader to guide and support our Aged Care community team of Support Workers while helping older people live independently and confidently in their own homes.
This is a hands‑on leadership role where you'll spend your time coaching staff, supporting quality care, building strong relationships and helping your team deliver their best work every day.
Based in the East Metro, this role offers a mix of community-based work, time in our Rise offices (including Helen's Place in Middle Swan), and work-from-home flexibility. Travel throughout the region is required. A 9-day fortnight may be available in the future.
What You'll Be Doing
- Leading, mentoring and developing a team of Support Workers to deliver high-quality, person-centred care
- Training, onboarding and coaching team members through regular check‑ins, buddy shifts and performance conversations
- Conducting visits, spot checks and competency assessments to maintain quality, compliance and protected work practices
- Working closely with Client Coordinators to respond to changing client needs, feedback and service outcomes
- Supporting recruitment, workforce planning and team engagement initiatives
- Providing direct client support when needed and building positive relationships with clients and their families
The need-to-have
You're a people-focused leader who enjoys developing others and creating positive outcomes for both clients and employees. You bring a practical approach, strong communication skills and a genuine commitment to quality care. Ideally, you'll have:
Skills
- Experience leading and developing teams in aged care, home care, community services or a similar environment
- Certificate III in Ageing Support, Community Services or equivalent industry experience
- Knowledge of Home Care Packages, aged care standards and relevant legislation
- Strong interpersonal, administrative and computer skills
- The ability to work autonomously while managing a community based workforce
- A passion for person-centred care and supporting older people to live well at home
