About The Role
- The role involves delivering patient centred care that coordinates individualised care from admission and throughout hospitalisation, incorporating discharge planning in the plan of care.
- Includes provision of detailed documentation of a systematic comprehensive and accurate assessment of patients.
- Coordinates and implements the intervention(s) identified in the plan of care/ clinical path.
- Evaluate and record progress towards expected outcomes and revise the plan of care as necessary.
- Innovative and informed by current research using contemporary techniques and technology.
About You
Essential Criteria
- Must be a Registered Nurse with AHPRA.
- Minimum 1-2 years clinical experience as a Registered Nurse with some cardiac experience.
- Demonstrate high level of interpersonal skills.
- Must work well within a team environment and promote team cohesion.
