Key Responsibilities
About you:
This is a rewarding role offering the opportunity to make a real difference in people’s lives. We are looking for:
- Minimum 1 year experience in community services or social support
- Robust communication and interpersonal skills
- Ability to work independently and as part of a team
- Diploma of Community Services (or equivalent)
- A compassionate, reliable, and proactive attitude
- Ability to build positive relationships with clients and stakeholders
- A commitment to promoting inclusion and independence
Skill & Experience
Tasks & responsibilities:
- Advocate for clients’ rights, access, and inclusion within the community
- Provide person-centred support tailored to individual needs
- Develop and implement individualised support plans
- Support clients with community engagement and participation
- Promote independence, wellbeing, and life skills
- Refer clients to appropriate specialist services and agencies
- Maintain accurate documentation and progress notes
- Work collaboratively with team members and external providers
- Be part of a supportive and values-driven team
