Monash Health is seeking a Credentialled Diabetes Nurse Educator for a part-time position across community sites including Cranbourne Community Hospital. You will provide diabetes education, evidence-based, patient-centred care and liaise with a multidisciplinary team.

The role focuses on assessment, education and coordination of clinical care, with emphasis on leadership and ongoing career development within Monash Health’s CART Proactive Team.
