Service Coordinator - NDIS/NIISQ & Aged Care
The Service Coordinator is responsible for ensuring that NDIS and Support at Home clients in their region receive timely, effective, and high-quality services from Quick Response Carers. This is a frontline leadership role focused on high standards, relationship management, and exceptional customer service. You are the Manager for Bundaberg Region, and responsible for daily operations and service delivery across our NDIS and Aged Care programs, ensuring best practice and participant satisfaction. You will manage and oversee a small team of Support Workers. You will work from a large, serviced office in the CBD, where clients and workers will often come to see you in person.
Key responsibilities
Undertake community engagement activities designed to raise awareness of Quick Response Carers and our service offerings.
Serve as the primary point of contact for participants, clients, families, and advocates.
Advertise, nurture enquiries and referrals, and enter into Service Agreements with clients.
Design and coordinate rosters based on package funding and client choice.
Match support workers to clients, taking into account skills, preferences, and needs.
Utilise the capabilities and strengths within your team to actively engage clients about their goals, seek local opportunities of interest and to participate in meaningful activities. Oversee the development of individuals write Support Routines and Care Plans.
Provide Case Management to Support at Home clients (Aged Care).
Ensure the teams service delivery is held to high standards and client satisfaction.
Ensure continuity and responsiveness of services, including emergency or short-notice situations.
Conduct regular service reviews to ensure alignment with our Policies and Procedures.
Perform after hours on-call duties as part of a rotating roster.
Utilise our CMS ''Shiftcare'' for efficiencies in billing and records management.
About you
Solid understanding of NDIS and Aged Care operational and service delivery requirements.
Experience and success in team leadership and supervision.
High-level interpersonal and relationship management skills.
Excellent organisational and time management abilities.
Driver's license and car.
NDIS Worker Screening Check.
Relevant Diploma or higher in Community Services, Disability, Aged Care, or relevant experience (desirable).
Familiarity with rostering and case management systems (desirable).
