Clinical Care Partner – Community Services – Gorges Hall (On-site)

n

Salary: $85,000 – $110,000 + super

n

Salary Packaging: Up to $15,900 tax-free + additional $2,650 per year (meals & entertainment)

n

Key Responsibilities

n

n
- Conduct comprehensive client assessments and develop care plans
n
- Coordinate in-home services and liaise with external providers
n
- Provide education, guidance, and advocacy for clients and families
n
- Monitor care budgets and ensure compliance with aged care standards
n
- Build strong, trusted relationships with clients and stakeholders
n

n

About You

n

n
- Current AHPRA Registered Nurse registration
n
- Experience in aged care, community nursing,



or care management
n
- Sound knowledge of the Support at Home Program and aged care standards
n
- Strong communication, organisation, and time-management skills
n

n

Requirements

n

n
- Current NSW Driver's Licence with a reliable, insured vehicle
n
- Right to work in Australia
n
- Valid AHPRA registration
n
- Pre-employment medical, police check, and required vaccinations
n
