About the role

n

As a Care Partner, you'll work alongside older Australians to understand what's important to them and coordinate the supports that help them live independently and achieve their goals.

n

You'll build trusted relationships, complete assessments and reviews, develop care plans, manage individual budgets and make balanced decisions that consider participant choice,



risk and available funding.

n

About you

n

You'll be someone who combines empathy with sound judgement.

n

You'll bring:

- Experience in aged care case management or care coordination
n
- Excellent communication and relationship-building skills
n
- Strong attention to detail
n
- Confidence managing budgets and balancing participant goals, risk and sustainability
n
- A cooperative approach and commitment to delivering high-quality, person-centred care
n
