About the role

Elka Home Care is seeking an experienced and compassionate local Care Partner to support older Australians across Bendigo and surrounding areas.

n

You will manage a portfolio of Support at Home clients, acting as their trusted local contact and coordinating services that help them remain safe, independent and connected at home.

n

This is a care management and coordination role, not a direct support worker position.

n

Key responsibilities

- Manage a portfolio of Support at Home clients.
n
- Conduct home visits, assessments and care plan reviews.
n
- Coordinate support workers, nursing, allied health and community services.
n
- Build relationships with clients, families and local providers.
n
- Build and monitor client budgets, funding and service utilisation.
n
- Respond to changes in client needs, concerns and risks and reflect in their care plan.
n
- Build and maintain accurate care plans, case notes and client records.
n
- Support new clients through onboarding and commencement of services.

What we're looking for

n

n
- Experience in aged care case management,



care coordination or community services.
n
- Knowledge of Home Care Packages or the Support at Home program.
n
- Solid communication, organisation and relationship-building skills.
n
- A compassionate and person-centred approach.
n
- Confidence managing client budgets and complex care needs.
n
- Relevant aged care, nursing, community services, allied health or social work qualifications are highly regarded.
n
- Current Police Check, driver's licence and reliable vehicle.
n
