Support at Home Clinical Care Partner

Lumia Care is seeking an experienced and compassionate Clinical Care Partner to join our in‐home aged care team in Hawthorn, Melbourne. This is a key role supporting older Australians to remain independent, safe, and respected in their own homes.

About the Role

n

As a Clinical Care Partner, you will work autonomously in the community while collaborating with a dedicated and supportive team. You will coordinate personalised in‐home care for participants funded under the Australian Government's Support at Home program.

n

This role combines clinical expertise, care coordination, and stakeholder engagement to ensure high‐quality, compliant, and person‐centred service delivery.

n

Key Responsibilities

- Conduct home visits to assess participant needs, including safety and mobility assessments, and develop individualised care plans
n
- Build strong relationships with participants, families, and health professionals to deliver tailored, outcome‐focused care
n
- Coordinate services, manage handovers, and ensure care plans, budgets, and supports are accurately documented and delivered
n
- Maintain compliance with the Aged Care Act 2024, relevant standards, and internal policies
n




- Respond to participant feedback and complaints, ensuring timely resolution and positive outcomes
n
- Support continuous improvement initiatives, including quality assurance, policy development, and accreditation activities
n
- Attend the office weekly to collaborate with the team and contribute to operational alignment
n

What You Bring

n

n
- Registered or Enrolled Nurse with minimum 3 years' experience in aged care, community, or health services
n
- Current AHPRA registration and NSW Driver's Licence
n
- Robust understanding of Commonwealth in‐home aged care programs, legislation, and standards
n
- Well‐developed clinical judgement and stakeholder management skills
n
- Confidence using multiple systems, portals, and Microsoft Office
n
- A genuine commitment to delivering high‐quality, participant‐centred care
n
