At Levande, wellbeing is about helping residents access the right support, maintain independence and continue living well within their community.

As Resident Wellbeing Manager, you'll work across our Victorian portfolio, partnering with village teams and Home Care providers to deliver consistent wellbeing outcomes and support residents through changing care needs.

n

This role is ideal for someone with Home Care or community care experience who enjoys building partnerships, influencing outcomes and making a meaningful difference.

What you'll be doing

n

n
- Supporting villages to deliver Levande's Everyday Wellbeing program
n
- Managing Home Care provider relationships, referrals and service outcomes
n
- Acting as a key escalation point for resident wellbeing matters
n
- Supporting residents through changing care needs and wellbeing transitions
n
- Using wellbeing data and insights to identify opportunities for improvement
n
- Leading wellbeing initiatives and projects across the Victorian portfolio
n

About you

n

You'll bring:

n

n
- Experience in Home Care, Community Care, Retirement Living or Aged Care
n
- Strong understanding of Home Care Packages and My Aged Care
n
- Experience managing provider relationships, referrals or care coordination
n
- Robust stakeholder engagement and communication skills
n
- Experience leading programs, projects or operational initiatives
n
