Job Description
Hand in Hand Disability Support – Kallangur QLD

We are currently looking for Disability Support Workers to join our team to work with participants across North Brisbane. You will be working within a supportive and professional environment, providing meaningful assistance to people within your community to enhance their independence and quality of life. You will be part of a cooperative team that values ongoing training, professional development, and best-practice service delivery.

We are seeking experienced Disability Support Workers who are committed, adaptable, and person-centred in their approach. Ideal candidates will demonstrate the ability to think creatively, work proactively, and support participants to achieve both their current and long-term goals. Applicants must hold a minimum Certificate III in Individual Support or Community Services and have at least one year of relevant industry experience.

Hand in Hand Disability Support delivers in-home support, Supported Independent Living (SIL), short-term accommodation, and community access services to people living with disability. Supports are provided across a seven-day rotating roster, including day, evening, overnight, active overnight, and sleepover shifts. Applicants must be flexible and available to work across a 24-hour care environment.

Duties and responsibilities may include:
n
n
- Personal care: showering, dressing and grooming
n
- Medication assistance
n
- Supporting participants with behaviours and following a Positive Behaviour Support Plan
n
- High Intensity supports including PEG / Enteral feeding, Ventilation Support, Complex Bowel Care, Catheter Support, Epilepsy, Subcutaneous Injections / Diabetes Support,
Complex Wound care Dysphagia Support including assistance with Mealtime Management.
n
- Domestic assistance: light household cleaning, laundry and meal preparation.
n
- Community Access: social activities, grocery shopping.
n
- Assistance in travelling to medical appointments.
n
- Supporting participants in the use of mechanical aids and personal equipment including wheelchairs, manual or electric hoists, slide sheets and moving boards.
n
- Supporting participants who have mental health challenges.
n
- Supporting participants with complex and/or challenging needs.
n
n
Experience with high intensity supports is preferred but not essential as training will be provided.
n
Expected to follow the company policies and procedures and code of conduct.
n
Monitoring the well-being of participants reporting and documenting on their progress.
n
Attending training as required.
n
Be asked to work anytime over a 24-hour period 7 days a week, this may include sleep over shifts or awake shifts.
n
Candidates must be available to work full seven-day coverage including weekends, public holidays, and across all shift types as required by the 24-hour care environment.

You will have:
n
n
- Minimum Certificate III or IV Disability or Community Services OR Minimum of 1 years' experience in disability support work
n
- Current CPR / First Aid Certificate
n
- Current and Valid Blue Card & NDIS Worker Screening Check
n
- Current Australian Driver's License
n
- Reliable, Registered and Insured vehicle
n
- Smartphone with internet access
n
- NDIS Worker Orientation Module
n
- Australian work rights are required for this position
n
