Job Description

Holicare is seeking a Program Manager to strengthen referral networks and grow Support at Home and NDIS services across Victoria. You will conduct client intake, home visits and assessments, develop person-centred care plans, and coordinate service delivery to ensure quality and budgets stay within funding guidelines.

With experience in aged care, disability or community services, you will build skilled networks, manage KPIs, and travel locally to support communities.
