Role Overview
n
As the Clinical Care Coordinator, you'll coordinate services for clients on Trilogy Care's Restorative Care and End-of-Life pathways. Working alongside Care Clinicians, you'll support care plan implementation, liaise with clients and providers, and ensure a seamless, person-centred experience, allowing clinicians to focus on delivering clinical care.

What You'll Do
n
n
- Coordinate client admissions, onboarding, reviews, appointments and transitions across the Restorative Care and End-of-Life pathways.
n
- Maintain regular communication with clients, families, representatives and service providers to ensure a seamless, person-centred experience.
n
- Coordinate restorative programs, allied health and community provider referrals, equipment, support services, discharge planning and bereavement follow-up where required.
n
- Coordinate provider services, monitor service commencement, review timeframes and pathway budgets, support My Aged Care processes, assist with funding requests and escape delays.
n
- Working under the direction of the Care Clinician,
complete operational follow-up actions, coordinate outstanding referrals, monitor review dates, escalating client deterioration and support implementation of care plans.
n
- Maintain accurate documentation, case notes, CRM records, clinical outcome data and pathway KPIs.
n
- Comply with workplace health and safety requirements and contribute to a protected, supportive and psychologically healthy workplace.

What You'll Bring
n
n
- Case management experience in aged care or NDIS.
n
- Experience within Support at Home or Home Care programs.
n
- Demonstrated ability to effectively work within a team with positive client outcomes.
n
- Compliance with Trilogy Care pre-employment and ongoing compliance requirements.
n
- Current National Police Check (and international check where applicable).
n
- Evidence of Australian work rights.
n
