About the Role

n

HYPAR are looking for exceptional support workers/mentors to assist our participants to achieve their goals through leading and mentoring, and building relationships based on HYPAR's core values.

n

Everyday can be a little different, but here at HYPAR the role of a support worker/mentor includes:

n

Implementing care plans, safety plans and behaviour support plans.

n

Aligning supports with allied health professional's recommendations to assist with achieving goals and using best practice strategies.

n

Planning, assisting and engaging participants in suitable activities.

n

Support participants to engage community and mainstream service.

n

Provide domestic support, including but not limited to; house cleaning, laundry, cooking, budgeting, shopping, etc.

n

Provide mental health support within a therapeutic framework to deliver quality care.

n

Complete high-quality documentation to provide the NDIS and governing bodies with information relating to supports, incidences and funding needs.

n

Positions are available throughout the Moreton Bay region.

About You

n

The ideal candidate will have:

n

Have previous experience working with people with complex disabilities and behaviours.

n

Have experience working with people with psychosocial disabilities

n

Exceptional emotional intelligence, demonstrating initiative and flexibility, being able to build strong relationships with participants, families and stakeholders.

n

The ability to maintain professionalism in high-pressure situations.

n

Excellent communication skills – both written and verbal, and are proficient in the use of smart phones and computers.

n

Robust professional boundaries, and the understanding of their importance.

n

A drive to do purposeful work that improves the quality of life and goal outcomes for out participants.

n

Ability to work a flexible roster, with availability in the mornings, evenings, and weekends. Preference to have availability to do 1-2 sleep over shifts per week. We are also looking for a mentor for overnight awake shifts.

n

Knowledge of NDIS Guidelines for reasonable and necessary supports, and the NDIS Commission Quality and Safeguarding Framework.

n

You will also be required to have or be willing to obtain:

n





Minimum Certificate IV or higher in a relevant qualification - Highly regarded

n

Disability Support Work or industry relevant experience - Highly Regarded

n

Paid Blue Card

n

Paid NDIS Disability Worker Screening (or valid yellow card)

n

Child Safety background screening check (LCS2)

n

Have access to a registered vehicle and full comprehensive insurance

n

Open QLD Drivers Licence (International and P-Plate Licences will not be accepted)

n

A working android or apple smartphone

n
