Vietnamese Case Manager (Aged Care+ NDIS)

Home Caring is a reputable and quick-growing provider of NDIS and Aged Care services, nationwide. We are currently seeking an experiencedVietnamese speaking Case Manager. The Case Manager will support a growing number of clients in the Smithfield and surrounding area. This role is a hybrid role and is a mix of WFH traveling to clients in the community and in the office coordinating associated administration and engagement events.

This role is ideal for someone who is community-minded, culturally sensitive, and committed to providing high-quality, person-centred care. You'll be working with clients across both Aged Care and Disability sectors.

Key Responsibilities:

- Conducting client assessments and developing person-centred care/support plans
n
- Coordinating services,



monitoring outcomes, and reviewing care plans regularly
n
- Advocating for clients and ensuring their cultural, linguistic, and health needs are met
n
- Supporting clients to access appropriate services and resources
n
- Liaising with families, support workers, and health professionals
n
- Ensuring compliance with the NDIS & Aged Care quality standards
n
- Enrolling and management of Home Care Packages for Aged Care clients and NDIS funding for Disability clients
n

What We're Looking For:

n

n
- Qualification in and/or relevant Case Management experience.
n
- Fluency ina Vietnamese and English are necessary.
n
- Experience in aged care, disability, or community health settings.
n
- Strong communication, problem-solving, and organisational skills.
n
- Understanding of NDIS and/or Aged Care systems.
n
- Empathy, cultural awareness, and a passion for helping others.
n

- Current First Aid & CPR, National Police Check, and NDIS Worker Screening
n
- Driver's Licence and access to reliable vehicle.
n
