As a Care Manager, you'll be at the heart of what we do – helping clients live well at home while ensuring high-quality, person-centred care. This role isn't just about coordination; it's about connection, strategy, reablement and making a difference. You'll champion client outcomes and contribute to business success by building trust, identifying unmet needs, and helping us bring Dovidas relationship-based care to more people across our Redlands region.

You'll collaborate closely with our passionate office team and national network, with the flexibility to work from home when on the road and in the office.

Key Responsibilities

- Build strong, trust-based relationships with clients and families
n
- Lead client onboarding, reviews, and ongoing care coordination through a wellness and reablement lens
n




- Develop and formalise individualised care plans and service agreements
n
- Actively promote Dovidas services to ensure high-quality support and stronger business outcomes
n
- Provide ongoing feedback and collaboration with our Caregiver team
n
- Document interactions and keep all stakeholders informed
n

About you

n

n
- Previous experience in case management, ideally in aged care or Home Care Packages/ Support at Home
n
- Outstanding relationship-building and communication skills
n
- A confident, professional approach to client engagement and service promotion
n
- Experience with Microsoft Office Suite - especially Outlook, Excel, and Word
n
- Solid organisation, time management, and documentation skills
n
- Valid Australian Driver's Licence and willingness to travel across to our Birkdale Office and out to Logan and surrounds
n
- Passion for empowering older Australians to live independently and with dignity
n
