About the Role
n
We're looking for an experienced Part-Time Chronic Disease Management (CDM) Nurse to join our Leichhardt Medical Centre.
n
In this rewarding role, you'll work closely with GPs and patients to support the prevention and management of chronic conditions. You'll play a vital role in empowering patients to take control of their health, improving long-term outcomes, and reducing avoidable hospital admissions.
n
Key Responsibilities
n
n
- Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
n
- Conduct health assessments and identify patients who may benefit from chronic disease management.
n
- Support and educate patients to better understand and manage their health conditions.
n
- Coach patients on sustainable health behaviour changes.
n
- Coordinate referrals and ongoing care with multidisciplinary healthcare providers.
n
- Partner with GPs to promote preventative healthcare initiatives across the practice.
n
- Increase patient engagement in chronic disease management programs.
n
- Ensure care plans are reviewed and updated in line with clinical requirements.
n
- Complete all administrative tasks associated with CDM appointments accurately and efficiently.
n

About You
n
To be successful in this role, you will have:
n
n
- Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
n
- Demonstrated experience in Chronic Disease Management within a general practice setting.
n
- Experience completing health assessments (highly regarded).
n
- Excellent organisational and time management skills.
n
- Strong communication and interpersonal skills with a patient-focused approach.
n
- A genuine passion for preventative healthcare and improving patient outcomes.
n
