Key Responsibilities
n
Provide case management and support services to clients,
including needs assessment and referral to appropriate services.
n
Develop, implement, and evaluate community programs and initiatives.
n
Advocate for clients to access housing, health, education, and employment services.
n
Build and maintain strong relationships with community stakeholders, service providers, and government agencies.
n
Maintain accurate client records and prepare reports in line with organisational and funding requirements.

To be successful in this role, you will have:
n
A relevant qualification in Community Services, Social Work, or a related field (minimum Diploma).
n
Demonstrated experience working with diverse communities, including culturally and linguistically diverse groups, Aboriginal and Torres Strait Islander peoples, and individuals experiencing disadvantage.
n
Strong knowledge of community services, referral pathways, and social support systems.
n
Excellent communication, advocacy, and interpersonal skills.
n
Ability to work independently and as part of a team.
n
Current Working With Children Check and National Police Clearance (or willingness to obtain).
