Experienced Support Coordinator – Fraser Coast (Part-Time)

Future Focus Support Coordination is seeking an experienced Support Coordinator to join our Fraser Coast team. About the Role

n

Working across the Fraser Coast region, you'll support NDIS participants to:

n

n
- Implement and maximise their NDIS plans
n
- Build strong support networks
n
- Navigate disability, health, housing, mental health, justice and community systems
n
- Strengthen their independence and decision-making
n
- Access the right services at the right time
n

n

This is a hybrid role with flexibility to manage your own diary, work from home, and conduct participant meetings across the region.

n

You'll be supported by experienced leaders, regular supervision, and a collaborative team environment that understands the realities of Support Coordination.

n





We're Looking For Someone Who:

n

n
- Has previous experience as an NDIS Support Coordinator
n
- Is positive, adaptable and solutions-focused
n
- Remains calm and resilient when supporting complex situations
n
- Enjoys working independently while still being part of a team
n
- Builds strong relationships with participants, families and providers
n
- Understands the importance of participant choice, control and dignity
n
- Has strong written and verbal communication skills
n
- Lives locally within the Fraser Coast region
n

Essential Requirements

n

n
- Previous Support Coordination experience
n
- Relevant qualification in Community Services, Social Work, Psychology, Mental Health, Allied Health or a related field (or substantial industry experience)
n
- Current Driver Licence
n
- Reliable vehicle
n
- NDIS Worker Screening Check
n
- Working with Children Check (or ability to obtain)
n
- Ability to satisfy relevant probity requirements
n
