Job Description
Provide culturally safe counselling, assessment, crisis intervention and case management services.
n
Develop, implement and monitor individual care plans in collaboration with clients, GPs and multidisciplinary teams.
n
Support individuals, families and communities experiencing mental health concerns, social and emotional wellbeing issues, grief, trauma and other life challenges.
n
Deliver early intervention, prevention and health promotion activities that enhance community wellbeing.
n
Build and maintain effective partnerships with local health providers, community organisations and government agencies.
n
Maintain accurate client records and documentation using Communicare and other organisational systems.
n
Participate in case conferences, multidisciplinary meetings and continuous quality improvement activities.
n
Contribute to achieving service delivery targets, funding requirements and key performance indicators.
n
Promote culturally appropriate, trauma-informed and strengths-based approaches in all aspects of service delivery.
n
Travel across the Goondir service region as required.
n
What We're Looking For
n
Tertiary qualification in Psychology, Social Work, Mental Health, Counselling, Welfare or a related discipline.
n
Eligibility for membership or registration with an appropriate qualified body (e.g. Australian Counselling Association or equivalent).
n
Experience providing counselling and case management services within community, primary health or mental health settings.
n
Demonstrated understanding of Aboriginal and Torres Strait Islander cultures and commitment to delivering culturally safe care.
n
Excellent interpersonal, communication and relationship-building skills.
n
Ability to work collaboratively within a multidisciplinary team while managing an independent caseload.
n
Strong organisational skills with the ability to maintain accurate clinical records and meet reporting requirements.
n
Current Queensland Driver Licence.
n
Current Blue Card, National Police Check (or ability to obtain).
n
Current CPR and First Aid Certificate (or willingness to obtain).
n
Experience working within an Aboriginal Community Controlled Health Organisation (ACCHO).
n
Knowledge of Communicare or similar electronic medical record systems.
n
Experience in mental health promotion, community engagement and group program facilitation.
